Job summary

MyCHN is a nonprofit FQHC serving Greater Houston, dedicated to improving lives through compassionate, patient-centered care. As a Behavioral Health Therapist, you will deliver comprehensive behavioral health services for patients of all ages and coordinate with an integrated care team to support whole-person health.

You will conduct assessments, provide counseling (individual, couples, family, group) and collaborate with primary care providers to ensure timely, evidence-based care.

Qualifications

• Active and unrestricted Texas license as an LCSW or LPC.
• Eligibility for Medicare/Medicaid provider participation.
• At least one year of clinical experience in area of practice.
• Ability to practice independently.
• Strong collaboration and care coordination skills.

Responsibilities

• Conduct behavioral health assessments and develop individualized care plans
• Gather patient histories through interviews with patients and families
• Provide individual, couples, family, group, and crisis counseling services
• Support wellness management and coordinate referrals for specialized care when needed
• Collaborate with providers, psychiatrists, and interdisciplinary care teams
• Educate patients on behavioral health conditions and health-promoting activities
• Ensure timely and accurate documentation of all patient interactions
• Manage follow-up care and ensure timely communication with patients
• Support integrated care coordination across behavioral health, primary care, and community resources
• Perform additional duties as assigned within scope
