Licensed Clinical Social Worker (LCSW)

Senior Care Therapy seeks full‑time or part‑time LCSWs to deliver in‑person psychotherapy to the geriatric population across over 300 skilled nursing facilities in NJ, NY, PA, and MD.

Key Responsibilities
  • Conduct individual and group psychotherapy for patients in sub‑acute or skilled nursing facilities
  • Assess, diagnose, and treat emotional and behavioral health conditions (depression, anxiety, adjustment disorders, cognitive decline)
  • Develop and implement evidence‑based treatment plans tailored to each patient
  • Collaborate with facility staff, interdisciplinary teams, and family members to support patient care
  • Maintain accurate and timely clinical documentation in the clinician‑designed EHR
  • Monitor patient progress and adjust treatment plans as needed
Requirements
  • Active and unrestricted LCSW license in the state of application
  • Excellent oral and written communication in English
  • Basic proficiency with EHR and technology
  • Strong organizational and documentation skills with regulatory compliance focus
  • Ability to provide in‑person services at assigned facilities (up to 45 minutes per session)
  • Physical capability to walk, stand, and move between patient rooms and offices throughout the day
  • Full vaccination and booster status may be required at some facilities
Preferred Qualifications
  • Previous clinical experience in a sub‑acute, long‑term care, or geriatric setting
  • History of treating anxiety, depression, and adjustment disorders; psych or addiction experience is a plus
