Connections Health Solutions in King of Prussia, PA seeks a Licensed Mental Health Professional (CM III) to manage care for individuals in crisis, coordinate admissions, treatment planning, and discharge planning with an interdisciplinary team. Requires a Master’s degree in Social Work, Counseling, or related field and PA licensure (LSW or LAC), with 1–2 years in mental health.

BLS within seven days, mandatory vaccination, and credentialing.

Qualifications

• 1–2 years of experience in a mental health setting
• BLS (Basic Life Support) must be obtained within seven days of hire

• Complete all credentialing requirements (NPI, Medallion, Medicare) as required for the role
• Must pass background checks (Child Abuse Clearance, FBI, PA State Police, Medical screenings)

Responsibilities

• Conduct psychosocial assessments within specified timeframes
• Develop and implement individualized Treatment Plans
• Coordinate care with interdisciplinary team and discharge planning
• Gather collateral information for treatment and discharge planning
• Identify gaps and make recommendations for post-discharge needs
• Maintain accurate clinic records and treatment planning
• Maintain knowledge of PA state behavioral health system and 302 processes
• Participate in ongoing education and competency assessments
• Perform other duties as assigned

Education

What You'll Do:

The Licensed Mental Health Professional (CM III) is responsible for managing the care of individuals who are in crisis and stabilizing from crisis situations. This role involves facilitating admission intakes, treatment planning, discharge planning, obtaining collateral information , assisting with applicable legal processes , and initiating personalized care plans. The Licensed Mental Health Professional works closely with an interdisciplinary treatment team to coordinate care and discharge planning for individuals, their families, clinics, local providers, and other referral sources.

• Facilitate admission intakes, educating and informing individuals about consents for treatment and authorizations for use and disclosures.
• Conducta thorough,accurate,andclinically sound Psychosocial Assessment with theindividualwithin 72 hoursof admission to Walk- In or observation , and within 24 hours for individuals admitted to the CSU .Maintainthe clinic record, including documentation of activities performed as part of theservice deliveryprocess, and obtainsignatures fromindividualsregardingtreatment, as necessary.
• Collaborate with the interdisciplinary treatment team to develop and implement individualized, person-centered Treatment Plans suitable for brief intervention.
• Complete and review signed Treatment Plans on CSU with each individual and any involved parties to ensure understanding and agreement.
• Gather collateral information from individuals and involved parties to conduct a thorough assessment of needs for treatment and discharge planning.
• Actively coordinate care and provide referrals to ensure services are aligned with both facility staff and external providers.
• Identify and address gaps in service needs, making appropriate recommendations to meet individuals' needs upon discharge
• Collaborate with the interdisciplinary treatment team to engage with, monitor, and communicate with individuals, ensuring safety and meeting their needs during the discharge process.
• Maintain accurate clinic records, discharge planning, and treatment planning.
• Maintain a working knowledge of the Pennsylvania State behavioral health system and 302 involuntary legal processes.
• Participate in ongoing education, including in-services, training, and other activities to maintain and improve competency.
• Demonstrate competency through post-testing, skill observation, and performance assessments as conducted by direct supervisors.
• Performs all other duties as assigned.

What You'll Bring:

• Master’s degree in Social Work, Counseling, Marriage and Family Therapy or a related behavioral health field, and current Pennsylvania licensure in good standing as one of the following:
  • Licensed Social Worker (LSW)
  • Licensed Associate Counselor (LAC)
• 1-2 years of experience in a mental health setting
• BLS (Basic Life Support) must be obtained within seven days of hire
• The Company has a mandatory vaccination policy. All successful applicants must be fully vaccinated, including showing proper documentation, or otherwise be exempt pursuant to the Company’s exemption process prior to their start date as a condition of employment
• Complete all credentialing requirements within the required time period, including but not limited to obtaining a National Provider Identifier (NPI), Medallion enrollment, Medicare enrollment, and any other necessary credentials as required for the role
• Must pass the following background checks

  • FBI Criminal Background Check
  • Pennsylvania State Police Criminal Record Check
  • Medical Screenings (Physical, TB, and Drug Test)

It would be great if you had:

• American Society of Addiction Medicine (ASAM) certification
• Current Pennsylvania licensure in good standing as one of the following:
  • Licensed Professional Counselor (LPC)
  • Licensed Clinical Social Worker (LCSW)
  • Licensed Marriage and Family Therapist (LMFT)
• Experience in acute mental health crisis settings
