Qualifications

• Current State License as a Licensed Practical Nurse, LPN.

Responsibilities

• Screens appropriate referral and coordinates admissions.
• Markets area for awareness of SOURCE and meetings.
• Completes new admission paperwork and in-home assessments.
• Participates in weekly team meetings with Medical Director.
• Completes quarterly home and PCP visits.
• Coordinates with therapy centers and home health agencies.
• Uses vehicle for travel and completes mileage sheets.
• Attends required trainings and trains new hires.
