Qualifications
Current State License as a Licensed Practical Nurse, LPN.
Must have a valid driver's license and automobile insurance liability.
Experience in hospice or SOURCE care preferred.
Responsibilities
Screens appropriate referral and coordinates admissions.
Markets area for awareness of SOURCE and meetings.
Completes new admission paperwork and in-home assessments.
Participates in weekly team meetings with Medical Director.
Completes quarterly home and PCP visits.
Coordinates with therapy centers and home health agencies.
Uses vehicle for travel and completes mileage sheets.
Attends required trainings and trains new hires.
