The role includes delivering psychoeducational interventions to prevent such conditions and offering consultation to individuals, couples, families, groups, and organizations. The LBHP will play a key role in supporting behavioral health needs through compassionate, evidence-based care.**Essential Functions:*** Facilitates and coordinates Individualized Service Plans (ISP) with patients and their families or caregivers, supporting consumers in articulating personal goals and objectives.* Proactively assists consumers and families in managing or preventing crises through planning and intervention.* Serves as the primary coordinator of behavioral health services, ensuring linkage to appropriate community resources.* Supports the development and management of an individual caseload.* Conducts therapy sessions and performs comprehensive clinical assessments.* Prepares discharge and transition documentation as needed.* Participates in team lead meetings and clinical supervision.* Conducts qualitative audits of medical records to ensure compliance and quality.* Provides assistance to consumers seeking help via phone or in-person visits.* Supports staff during consumer crises by helping complete accurate and timely documentation.* Authorizes services within the required electronic systems.* Facilitates group counseling sessions.* Performs behavioral assessments and develop individualized treatment plans.* Performs additional duties as assigned by the CMO.## Qualifications**Qualifications:*** **Education:** Master’s or higher degree in social work, counseling, marriage and family therapy, or a closely related behavioral health field.* **Licensure**: Active and unrestricted Georgia licensure as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT)* **Licensure**: Continuing (CME) credit to maintain license and board certification after residency.* **Experience**: Two years of acute Behavioral Health experience* Strong understanding of mental health practices, ethical standards, and regulatory requirements.* Excellent interpersonal, communication, and teamwork skills.* Demonstrated ability to manage multiple projects and lead diverse teams effectively.
