Covenant Medical Group is seeking a License Behavioral Health Counselor for the CMG Resident Program at West Mall Medical Park. The role is part-time (48 hours per pay period) and day shift, providing professional social work and counseling services across Covenant Medical Group clinics and referrals to community resources.

The position requires a master's in social work or counseling, two years of related experience, and licensure as LCSW, LPC, or LMFT.

Qualifications

• Master's degree from an accredited graduate social work or counseling program.
• Two years of related field work experience.
• Licensed as LCSW, LPC, or LMFT.

Responsibilities

• Assist patients/families with physical, emotional, and social concerns during care episodes.
• Provide treatment and discharge planning for patients.
• Document treatment plans, safety plans, and progress notes.
• Complete initial assessments and record social determinants of health.
• Connect patients with community resources and coordinate care.
• Liaise with adult/child protective services as needed.
• Collaborate with staff and residents; update medical records.
• Demonstrate empathy, communication, organization, critical thinking, active listening, and patience.
• Participate in warm handoffs with faculty and residents.
• Follow policies; complete education; participate in quality initiatives.
• Perform other duties as assigned.
