Qualifications

• Master’s degree with at least 3 years’ experience in counseling, social work or psychology.
• MI licensure prior to hire or transfer date (one or more specified licenses).
• BLS Provider preferred; American Heart Association acceptable.

Responsibilities

• Conducts patient assessments and establishes therapy treatment goals.
• Administers individual and group therapy and maintains records.
• Evaluates progress and effectiveness of therapy interventions.
• Provides information about resources and community activities.
• Collaborates with patient, family, physician and providers from treatment start to discharge.
• Engages in employee outreach and care coordination.
