Job summary

Life Care Services in Chandler, AZ is seeking an experienced LPN/Wellness Nurse for 12-hour shifts in our senior living community. You will administer medications, provide compassionate nursing care, and supervise the care provided by our team to ensure residents receive high-quality service.

The role requires an active Arizona LPN license, availability for emergencies, and a collaborative mindset to support daily operations on-site.

Qualifications

• Current LPN license valid in Arizona.
• Willingness to be available for emergencies.
• Yardley experience preferred but not required.
• Assisted Living and/or Memory Care Experience preferred but not required.

Responsibilities

• Assist residents with Activities of Daily Living (ADL) and promote independent living.
• Identify medical/behavioral changes and develop and monitor care plans.
• Provide daily reports to the Director of Health Services about conditions and needs.
• Provide nursing care per physician orders and established standards.
• Ensure preventative, therapeutic and rehabilitative services and coordinate care with other disciplines.

General Summary:

The LPN/Wellness Nurse is responsible for the overall nursing care/service on his/her shift, including the accurate and timely administration of medications, treatments, documentation, and emergency calls, as indicated. This position also provides direct nursing care to residents and delegates and supervises the care provided by our Quality of Life Specialists/Resident Assistants.

Essential Job Duties:

• - Assist residents with Activities of Daily Living (ADL), family/social activities and actively promote independent lifestyle of residents.
• - Responsible for identification of resident medical and behavioral changes and the development and monitoring of care plans and re-assessment of each resident.
• - Provide daily report to the Director of Health Services concerning community conditions, personnel performance, resident conditions, and needs for nursing supplies.
• - Provide nursing care according to physician’s orders and in conformance with recognized nursing practice, established standards, and administrative policies.
• - Ensure preventative, therapeutic and rehabilitative services to residents and assists in the coordination of nursing care with care rendered by other disciplines.
