Job summary

Clarendale of Chandler is seeking an experienced LPN/Wellness Nurse to oversee nursing care on 12-hour shifts and administer medications per physician orders. You will supervise and support the care team, respond to emergencies, and document accurately in each resident’s chart.

Arizona-licensed LPN required with on-call willingness. Experience in Assisted Living or Memory Care is preferred but not required; ongoing professional training and team collaboration are encouraged in a warm, upscale

Responsibilities

General Summary:

The LPN/Wellness Nurse is responsible for the overall nursing care/service on his/her shift, including the accurate and timely administration of medications, treatments, documentation, and emergency calls, as indicated. This position also provides direct nursing care to residents and delegates and supervises the care provided by our Quality of Life Specialists/Resident Assistants.

Essential Job Duties:

• Assist residents with Activities of Daily Living (ADL), family/social activities and actively promote independent lifestyle of residents.
• Responsible for identification of resident medical and behavioral changes and the development and monitoring of care plans and re-assessment of each resident.
• Provide daily report to the Director of Health Services concerning community conditions, personnel performance, resident conditions, and needs for nursing supplies.
• Provide nursing care according to physician’s orders and in conformance with recognized nursing practice, established standards, and administrative policies.
• Ensure preventative, therapeutic and rehabilitative services to residents and assists in the coordination of nursing care with care rendered by other disciplines.
