Job summary

Clarendale of Chandler is seeking an experienced LPN/Wellness Nurse to join our clinical team on a part-time basis. The role covers 12-hour shifts and involves administering medications, coordinating care, and responding to resident needs with professional nursing judgment.

Qualified candidates hold a current Arizona LPN license and are ready to handle emergencies, with prior assisted living or memory care exposure preferred.

Qualifications

• Current LPN license valid in Arizona.
• Willingness to be available for any/all emergencies regarding the community.
• Yardley experience preferred but not required.
• Assisted Living and/or Memory Care Experience preferred but not required.

Responsibilities

• Assist residents with Activities of Daily Living and promote independence.
• Identify medical/behavioral changes and update care plans.
• Provide daily report to the Director of Health Services on conditions and needs.
• Provide nursing care per physician orders and established standards.
• Ensure preventive, therapeutic and rehabilitative services and coordinate with other disciplines.

General Summary

The LPN/Wellness Nurse is responsible for the overall nursing care/service on his/her shift, including the accurate and timely administration of medications, treatments, documentation, and emergency calls, as indicated. This position also provides direct nursing care to residents and delegates and supervises the care provided by our Quality of Life Specialists/Resident Assistants.

Essential Job Duties

• Assist residents with Activities of Daily Living (ADL), family/social activities and actively promote independent lifestyle of residents.
• Responsible for identification of resident medical and behavioral changes and the development and monitoring of care plans and re-assessment of each resident.
• Provide daily report to the Director of Health Services concerning community conditions, personnel performance, resident conditions, and needs for nursing supplies.
• Provide nursing care according to physician’s orders and in conformance with recognized nursing practice, established standards, and administrative policies.
• Ensure preventative, therapeutic and rehabilitative services to residents and assists in the coordination of nursing care with care rendered by other disciplines.
