Qualifications

• Must possess a current California state license in good standing (PT, PTA, OTR, COTA or SLP).
• Minimum 2–4 years of clinical experience in a skilled nursing facility or similar setting.
• Previous supervisory or management experience in a rehabilitation department preferred.
• Strong knowledge of federal, state, and local regulations related to rehabilitation in SNFs.
• Excellent communication, interpersonal, and organizational skills; ability to work in an interdisciplinary team.

Responsibilities

• Assist the Director of Rehabilitation in planning, developing, organizing, and directing day-to-day rehab department functions.
• Supervise and support rehab staff including PTs, PTAs, OTs, SLPs, and therapy assistants.
• Ensure rehab services comply with standards, policies, procedures, and regulations.
• Develop, implement, and evaluate individualized care plans with the interdisciplinary team.
• Monitor budgets, resources, and supplies to optimize efficiency and cost-effectiveness.
• Lead staff meetings, provide ongoing training, and mentor rehab team members.
• Ensure accurate documentation of assessments, treatments, and progress notes.
• Participate in quality improvement initiatives and enhance resident outcomes and satisfaction.
• Maintain a safe and therapeutic environment for residents, staff, and visitors.
• Stay current with industry trends and regulatory changes to ensure compliance and competitiveness.
• Perform other duties as assigned by the Director of Rehabilitation.
