Family Healthcare of Hagerstown is seeking a Lead RN Case Manager to oversee case management for chronic illness patients under the PCMH framework. The role emphasizes education, self-management support, and timely care delivery within our center’s goals.

The Lead RN Case Manager coordinates workflow, guides staff, and ensures accurate documentation in the EHR/EDR, while maintaining professional nursing standards and patient dignity. Strong communication and independence are essential.

Qualifications

• Current RN license to practice in Maryland.
• CPR Certification.
• Prior Case Manager experience preferred.
• Proficient typing skills to enter data into the EHR efficiently.

Responsibilities

• Coordinate quality of care for patients with chronic illnesses.
• Collaborate with providers and staff to identify suitable cases for PCMH case management.
• Conduct initial and periodic assessments and document in EHR/EDR.
• Maintain and update case management plans and related documentation.
• Connect patients to community resources and ensure timely follow-up.
