Job Summary:

This position is located in San Felipe Pueblo, between Albuquerque and Santa Fe, and serves a community of approximately 3,500, in accordance with the requirements, goals, and objectives of the Pueblo of San Felipe Administration, the Indian Health Service P.L. 93-638 contract, and other contract requirements. Incumbent will serve as Lead Community Health and Wellness Nurse. The incumbent will plan, organize, and direct all functions related to public health in the community, including enforcement of all related laws, ordinances, and regulations; act as a public health consultant for the Pueblo of San Felipe and serve the community through work with other tribal programs/activities, home visits, epidemiological program management, infectious disease investigation, and community assessment skills.

Essential Duties: Leadership of Community Health and Wellness Nursing Program, Case Manager, and CHR Coordinator.

Administrative Duties:
  • Mentoring of New CH&W Nurses and Case Manager Program
  • Infrastructure Development, implementation, and evaluation. Scope of work, Policies, Procedures, Protocols, etc.
  • Development, implementation, and dissemination of the Pueblo of San Felipe Community Health Assessment every 3 Years
  • Develop and implement a program strategic plan.
  • Design and implement health education campaigns and disease prevention activities, such as immunizations and screenings
  • Collaborates and coordinates patient care services with Indian Health Service medical providers serving the San Felipe Community.
  • Participates in continuous evaluation of services and performance improvement.
  • Prepares and submits various patient and operational reports directed by the director.
  • Maintains confidentiality of all records.
  • Participates in continuous evaluation of performance improvement.
  • Knowledge of Health Services (IHS), ISDEAA 96-638, Federal Grants and Contracts
  • Prepare annual budget and monitor revenues and expenditures
Clinical Duties:
  • Provide direct primary, preventive, and therapeutic medical care appropriate to the age of patients served.
  • Makes home visits when indicated to implement a care plan and follow-up care.
  • Provides direct care for physical and psychological emergencies.
  • Provides direct health care services and behavioral health screening.
  • Sets local priorities for health-related interventions to provide the most significant benefit to the most people.
  • Assess and coordinate medical and behavioral health care interventions and plans.
  • Assess and coordinate wound care and podiatry.
  • Administer medication and immunizations within scope of work.
  • Conduct assessments and screens.
  • Performs medical screens, diagnostic tests, collection of tests, transport, and other medical procedures within their scope of practice.
  • Monitors health trends and identifies health risk factors unique to tribal communities.
  • Utilizes Standards of Nursing Practice and agency policies and practices.
  • Participates in the planning and coordination of various public health clinics in order to prevent communicable diseases and promote wellness.
Public Health Duties:
  • Educates people about available healthcare programs and services to improve access to care.
  • Conduct community analysis in partnership with community representatives and in collaboration with other public health or health care professionals.
  • Respond to Public health emergencies, testing, and care
  • Communicable disease screening, treatment management, and education.
  • Maintain confidentiality of all records.
Additional Responsibilities:
  • Other duties assigned within scope of practice.
  • Maintain current education and standards in Public Health Nursing with a primary focus on population-level outcomes and promote health and disease prevention for the entire population group.
  • Demonstrates computer skills necessary to organize data for essential care delivery.
Qualifications:
  • Education: Bachelor of Nursing or graduate -level nursing education in Public Health.
  • Experience/Basic Knowledge: 5 to 8 years of progressively responsible experience in public health, including extensive management and supervisory experience.
  • Proficiency in epidemiology, biostatistics, public speaking, conflict resolution, and L. 93-638 contract.
  • Verbal Communication, Health Promotion and Maintenance, Listening, Quality Management, Energy Level, Integrity, Infection Control, Informing Others, Nursing Skills, Medical Teamwork, Multi-tasking
  • Required Licenses/Certifications: Current RN license in the State of New Mexico, current basic CPR certification, and valid New Mexico driver’s license.
  • Physical Requirements: Must have manual dexterity to enter and retrieve data from a computer. Ability to sit and stand for long periods. Bends and stoops regularly.
  • Preferences: San Felipe Preference, AN/N and Veteran’s Preference.
