PositionSummary:


Lead Senior Instructor provides administrative day programsupports, to ensure documentation is completed by Instructors to show the progress or lack of progress towards meeting theindividuals' ISP goals for specific waiver services. Participates in all scheduled ISP meetings to ensure thedayprogram services meet the needs of theindividual.Conduct daily progressnotesaudits and monitorindividuals' treatment plan.Assistwith supervision and coordination. of departmentstaffand functions. The job requires 60% administrativework,30% direct care, and10% transportation, and other duties as assigned.

Supports Program Manager and employees through appropriate oral and written communication andpeople skills.   • Assistwith training new employees, as well as experienced workers in areas such as agency policy, department proceduresand .

  • Participate in developing and implementing agency administrative policy.

  • Supports in planning andexecuting dailyactivities ofindividualswith other staff and Program Manager.

  • Communicates regularly with the Program Manager.

  • Maintainrelationshipwith othercollaborativesto ensure that everyone is working towards the successful completion of the ISP goalsfor theindividual.

  • Activelylearnsabouttheinterests. Goals, preferences, supportneedsand skills of each person served through directinteraction, observation, insights of staff, team members, and family members

  • Actively attend allISP and SIS meetings as a representative of the agency, and ensures that all ISP's, HRST's, and BSP's are up to date and in case record.

  • Assistsin coordinating interdisciplinarytreatment teammeetings andassistin developing individualsISPs and assuresplans areimplemented by staff from multiple disciplines so thatidentifiedgoals are met andrequireddocumentation and trackingofISP goals is completed.

  • Effectively conducted periodic meetings (both regular and called meetings, as necessary) ofindividualsto make any changes necessary to theindividualsISP.

  • Directs interdisciplinary team toidentifyday program goals based onindividual'sneeds and dayprogramservices.

  • Disburse andpostsinformationregardingindividuals' treatment plans.

  • Monitors the implementation of individualindividualsplan through frequent rounds and observation, asnecessary.

  • Ensures collection andtimelysubmission of data, reports,formsand other pertinent information asrequiredto meet individual needs ofindividuals.

  • Effectivelymonitorstheindividuals' progress towards achieving goals, objectives, and amends plan as needed.

  • Ensuresindividualdocumentationandrecordsrequirementsarebeingmet. Conduct monthlyindividualrecordreviews;responsible for documenting outcomes and developing a plan of action.

  • Ensuresactivitynotesarebeingdocumentedtoshowprovisionofservices,asindicatedonthecurrentISPandcorrespondtogoalsonthecurrentISP.

  • Ensuresthat all requiredindividualssupportdocumentationiscorrectlyandaccuratelycompletedandsubmitteddaily.

  • Ensure the safety and well-being ofindividualswhile intheprogramcare.

  • Ensure safe transportation to individuals served by the Day Program

  • Accompany andtransportindividualstoparticipateinrecreationand community outings.

  • Maintainasafedrivingrecordandcomplywithalllocal traffic regulations.

  • Obtain 16 hours ofannual training as required byInCommunityStakeholders.

  • On duty for the hours assigned andimplement planned scheduled.

  • Attend regularly scheduled meetings.

  • Successfully completes Proxy Caregiver Training, both general training and work site specific training.

  • Other duties as assigned.


QualificationsEducation, Certifications & Experience Requirements


  • High School Diploma and sixmonths’experience in working in human services or relatedfield.
