Qualifications

• Bachelor’s degree required in a health field.
• Minimum of 5 years nursing experience.
• At least 1 year pediatric experience in a comparable setting.
• At least 1 year of demonstrated nursing leadership experience.

Responsibilities

• Provide clinical care coordination and continuity of care for a specific patient population.
• Lead patient care delivery teams and collaborate with physicians and other clinicians.
• Participate in performance improvement activities and safety initiatives.
• Coordinate across systems to ensure seamless care transitions for families.
• Engage in patient/family education and ensure documentation meets standards.
