Job summary

Complete Care in New Jersey is hiring a Lead Advanced Practice Provider (NP/PA) to guide NPs and PAs delivering outpatient and SNF palliative care across Bergen & Passaic. The role blends direct patient care with APP mentoring, quality oversight, and program-building.

The position offers protected leadership time and the chance to shape clinical standards while growing the palliative care program regionally.

Qualifications

• 5+ years across palliative care, hospice, geriatrics, oncology, internal/family/hospital medicine, SNF/LTC, or complex chronic care; palliative experience preferred.
• 1–2+ years of leadership, mentoring, precepting, team lead, or supervisory experience.
• Active New Jersey NP or PA license and national board certification.

Responsibilities

• Provide day-to-day leadership, mentoring, and clinical support to NPs and PAs.
• Carry a focused clinical caseload calibrated to team size — high-quality palliative care.
• Provide palliative care consultations and follow-up visits across SNF, subacute, and long‑term care settings.
• Lead goals-of-care and advance care planning conversations with patients and families.
