Job summary

• Maternal Child Health nursing experience preferred.
• Spanish as an additional language preferred but not required.

Responsibilities

• Provide preventive health and social services to at‑risk pregnant women and children birth to age two.
• Deliver education and connect families with community resources to promote health and development.
• Maintain a caseload of approximately 12–15 clients and establish therapeutic relationships per the NFP model.
• Conduct regular home visits with eligible pregnant clients per the NFP model.
• Coordinate with management and community health providers to support referrals and program guidelines.

Job description

The Lead Nurse Home Visitor provides preventive health and social services to at-risk pregnant women and children from birth to age two. This role uses advanced nursing and case management skills to assess family needs, screen for program eligibility, deliver evidence-based education, and connect families with community resources to promote positive health and developmental outcomes. The Lead Nurse Home Visitor collaborates closely with supervisors, private providers, and community agencies to support family functioning, ensure safe home environments, and achieve program outcomes that meet or exceed research-based benchmarks aligned with national standards.

Essential Duties and Responsibilities

Clinical Knowledge and Nursing Practice

• Demonstrate knowledge of public health principles, maternal-child nursing, and fundamentals of child growth and development.
• Provide teaching and promote education appropriate for nursing staff, including:
  • DANCE Integration
  • PIPE education
  • FAN education
  • New Hire Orientation
  • Team education modules, etc.
• Participate in Quality Assurance/Performance Improvement (QA/PI) activities.

Case Management and Client Support

• Promote linkages with health and human services on behalf of the program and clients.
• Maintain knowledge of community resources to facilitate referrals and access to services.
• Complete program forms accurately and keep client records up to date.
• Perform other job-related tasks as assigned.
• Maintain a caseload of approximately 12-15 clients, with a therapeutic relationship as established through the NFP model
• Conduct regular home visits with eligible pregnant clients in accordance with the NFP model

Collaboration and Liaison Responsibilities

• Serve as a liaison between Nurse Home Visitors, management staff, and community health and human service providers.
• Maintain positive working relationships with other organizations to enhance referrals and client access.
• Assist in developing program guidelines and policies that reflect evidence-based community strategies.

Lead Nurse Home Visitor Responsibilities

• Maintain strong working relationships and open communication between NFP staff and management.
• Coordinate with management to plan annual staff retreats.
• Orient new and existing staff to NFP and model program Fidelity.
• Efficiently operate DMCN and Apricot data systems and communicate updates to staff.
• Keep staff informed of changes in NFP requirements as discussed with the supervisor.
• Ensure up-to-date forms and materials are available for NFP Home Visitors.
• Serve as the point person for organizing Play Dates, graduation planning, and other program events.

Essential Qualifications

Education/Experience Requirements:

• Three or more years of service as a Nurse Home Visitor with GenerationEd
• BS in nursing and one year of Public Health experience preferred; or graduation from an accredited school of professional nursing.
• At least two years of professional nursing experience. Current license or temporary license to practice as a Registered Nurse in North Carolina by the NC Board of Nursing.
• Maternal Child Health nursing experience is preferred.
• Spanish as an additional language is preferred but not required.

Knowledge, Skills, and Abilities:

• Current, valid North Carolina driver’s license is required
• Certification in basic CPR

Physical, Mental and /or Visual Demands

• Must be able to sit and stand for extended periods of time throughout the day.
• Must be able to lift up to 35 lbs.
• Must be able to bend and stoop.
• The noise level in the work environment is usually moderate.
• Specific vision abilities required by the job include close, distance, color, peripheral depth and the ability to adjust focus.
