Job summary

Archbold Home Health, part of the UnitedHealth Group family, seeks a Home Health Patient Care Manager to supervise clinical services for a GA territory. You will coordinate an interdisciplinary team, ensure plan of care adherence, and manage physician orders and approvals with sensitivity to diverse patient needs.

Your role emphasizes leadership, regulatory compliance, and ensuring high-quality, individualized home health care within the community.

Qualifications

• Current unrestricted RN licensure in state of practice (GA).
• Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• RN licensure must have no restrictions in GA.

Responsibilities

• Supervise and coordinate clinical services with an interdisciplinary team.
• Receive referrals, assign clinicians, evaluate orders, and plot start of care visits.
• Coordinate home health benefits, medical necessity, and ongoing insurance approvals.
• Ensure care is individualized, compliant with physician orders and regulations.
• Review assessments and plans of care daily and consult clinicians as needed.
• Provide care in diverse home situations regardless of client background.
