Job summary

LSSI is hiring a full-time Lead Home Care Assistant, Monday through Friday, to help clients maintain independence through homecare and personal assistance and to assist the manager with program and administrative support.

Essential functions include on-call duties, updating schedules, maintaining client care records, following Plans of Care, assisting with daily living activities, and documenting hours of service with integrity.

Qualifications

• High school diploma or equivalent required.
• Bachelor's degree preferred.
• Background check clearance required.
• Minimum of one year experience in home care or comparable human services.
• RN/LPN/MD/physician assistant or CNA training preferred.
• CCP Home Care Aid training or equivalent preferred.
• Valid driver’s license; reliable transportation required; IL liability insurance required.

Responsibilities

• Perform on-call duties and manage on-call communications and schedule updates.
• Maintain timely communication with management and update client care records.
• Observe client functioning and report changes to supervisor.
• Assist with activities of daily living and household tasks per Plan of Care.
• Document daily activities and hours of service accurately and professionally.

Essential Functions:

• Perform on-call duties including but not limited to answering on-call phone in a timely manner, check and update schedules, cover shifts, resolve issues, and update manager.
• Maintain timely and accurate communication with management; report on and provide input into client's individual care record.
• Observe client functioning. Identify and report changed to the supervisor or employer.
• Follow client's written Plan of Care, including assistance with household chores, including but not limited to, cooking, meal preparation, cleaning, and laundry.
• Assist client with completing activities such as shopping and appointments outside the home and provide required receipts and documentation the conduct of essential shopping and/or errands.
• Assist client with activities of daily living and personal care.
• Compete appropriate records documenting daily activities, observations, and direct hours of service.
• Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest and fair manner.

Position Qualifications:

• High school diploma or equivalent required.
• Bachelor's degree preferred.
• Background check clearance required.
• Minimum of one (1) year experience in the home care field, comparable human services capacity, or experience caring for a dependent child or adult family member preferred.
• Documented, supervised training equivalent to eight (8) hours of homecare aide training within the past two (2) years preferred.
• Successful completion of RN, LPN, MD, physicians assistance or CNA training preferred.
• Previous experience as a CCP Home Care Aid preferred.
• Verified and documented current HCA completing LSSI designated Specialized training preferred.
• Demonstrated ability to show compassion for needs of the client and to be an active listener.
• Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing.
• Valid driver's license, in good standing for the state of residency required.
• Access to reliable transportation required.
• Valid IL statutory minimum liability insurance coverage, bodily injury and property damage required.
