Job summary

AbleLight in California is seeking a dedicated caregiver to deliver person-centered care in a residential setting, helping people we support achieve meaningful daily living, personal goals, and spiritual supports.

You will develop and implement Individual Support Plans, work with the team, document care and activities, and coordinate appointments, transportation, and community activities.

Qualifications

  • High School diploma or equivalent preferred, but not required unless directed by state regulations.
  • One year’s previous experience in a work position, volunteer/community role, or educational setting that focuses on serving and helping others.
  • 6 months or more previous leadership experience preferred (project management, team/task lead, supervisor, etc.)
  • Drivers’ licensure required if role involves driving others or required by state regulations.

Responsibilities

  • Deliver person-centered care daily in a residential setting aligned with the Individual Support Plan (ISP).
  • Coordinate programming and staff development to support individuals and ensure quality.
  • Assist with documentation in EMR, medical orders, and state requirements; support health-related processes.
  • Schedule transportation, appointments, and community activities; participate in meetings and training.

Job Summary

The primary responsibility is to deliver person centered care and services to those we support in a residential setting to fulfill our purpose for existing “We believe the world shines brighter when people with developmental disabilities achieve their full potential” Assists in the development and delivery of holistic individual support plans, assisting people in achieving their personal goals and desires, providing interaction and choices for activities in their community that support a meaningful day, assist with spiritual supports and encouraging individuals to be independent in activities of daily living and personal care.

The second responsibility is the development of activities and coordination of programming and services through staff development, individual support plans, and collaboration under the direction of the manager. While not a supervisor, this position is a resource for questions and guidance for others on day-to-day needs and provides back up to the manager.

65% Person Centered Care and Services

  • Ensure the ISP maximizes the quality of life, supporting engaging activities to enhance the quality of daily living, individuals’ personal goals, and spiritual supports.
  • Write and enter service delivery documentation, support others in entry of information into system.
  • Use AbleLight electronic medical record (EMR) and human resource technology to fully ensure timely, complete, and objective documentation aligned with medical orders, state requirements, and AbleLight quality and internal procedures and guidelines.
  • Ensures general event reporting (GER) is maintained and is aligned with medical orders, state compliance requirements, abuse reporting, court orders and AbleLight quality and internal procedures and guidelines.
  • Deliver holistic person-centered care daily care, with dignity and respect and aligned with the Individual Support Plan (ISP).
  • Provide personal care such as bathing, dressing, bathroom assistance or housekeeping.
  • Prepare and serve meals in accordance with individual diet plans.
  • Administer medication as documented under the direction (supervision) of a nurse.
  • Schedule and take individuals to medical appointments, outings, or errands.
  • Attend staff meetings and participati in continuing education.
  • Maintain a strong and collaborative working partnership with all colleagues, leadership, PWS, and parents & guardians.

35% Talent & Administration

  • Function as a front-line resource for DSP colleagues, persons supported, or parent & guardians in the home.
  • Bring recommendations forward related to quality and process improvements for our employees and those we serve. Escalates topics to the manager as needed.
  • In partnership with operational leaders, human resources, quality, and nursing:
      • Welcome, onboard, and train new colleagues and
    • raise additional training and development needs for staff.
  • For the identified residential home(s) coordinate and ensure standard operating procedures are consistently implemented including:
  • Audits of health appointments, individual support plans, monitoring medical supplies and prescriptions, and monthly facility audits.
  • Ordering food and supplies and allocation of receipts to house and program financials.
  • Reconcile and enter personal funds records.
  • Maintain a clean, safe, and healthy environment always in the residence.
  • Maintain a master calendar of appointments, facility activities, meetings for residence.
