Qualifications

  • Bachelor’s degree in social work, public health or related field; 2+ years in outreach and case management for high-risk populations.
  • Experience with stabilization, shelter, housing, and SUD treatment centers.
  • De-escalation, motivational interviewing, overdose reversal, data collection and case notes training.
  • Street-based outreach in urban settings and homeless encampments.
  • Ability to document case notes and data to professional standards.
  • Ability to collaborate across Glide programs and public agencies in the Bay Area.
  • Ability to engage clients with motivational interviewing and trauma-informed approaches.
  • Strong communication and public speaking skills.

Responsibilities

  • Oversee 24/7 shift operations and staff supervision for Journey Home.
  • Provide direct supervision and performance management for 1–3 Case Managers.
  • Verify relocations and reunifications for safety and documentation.
  • Safeguard client supplies, vouchers, and other valuables; conduct inventories.
  • Lead shift briefings, trainings, and team meetings; support staff development.
  • Oversee hotline response, triage, dispatch, and documentation; monitor KPIs.
  • Ensure compliance with safety protocols; serve as lead driver when needed.
  • Build relationships with CBOs, city agencies, and GLIDE programs.
  • Engage individuals experiencing homelessness; connect to shelter and wraparound services.
  • Document 50–60 active clients per case manager; submit timely reports.
  • Transport clients to appointments and manage resources with integrity.
  • Uphold HIPAA, confidentiality, and public communication guidelines.
