Lead Case Manager roles provide senior-level care coordination leadership, mentorship, and advanced caseload support, with opportunities to collaborate with clinicians, educators, employers, and treatment teams to strengthen wrap-around services

Responsibilities

• Direct Client Support: Deliver strengths-based interventions tailored to your focus area (classrooms, early childhood settings, workplaces, or community spaces).
• Care Coordination: Partner with clinicians, educators, employers, and treatment teams to ensure integrated, effective care plans.
• Resource Advocacy: Link clients and families to wrap-around services, community programs, and local support systems.
• Crisis De-Escalation: Provide trauma-informed support and de-escalation tailored to client and family needs.
• Documentation: Maintain accurate, timely progress notes in compliance with state and agency standards.
