On Lok has been a pioneer in delivering programs and services for the elderly, including founding the Program of All-Inclusive Care for the Elderly (PACE). Through On Lok Enhanced Care Management (ECM), On Lok serves the following populations of focus: Adults aged 21 and over at risk of long term care institutionalization, Adult nursing facility residents transitioning to the community, Adults At Risk of Avoidable Hospital or ED Utilization, and at‑risk seniors using coordinated care for OR care management and targeted interventions. The Lead Care Manager 1 will be responsible for coordinating health care resources, providing education, and facilitating access to care for a panel of members. The Lead Care Manager 1 serves as a clinical advocate for members, an active interdisciplinary team member, and a liaison with other health and social service providers in the community. Staff in this role may work with members who have mental health conditions and substance use disorders.
Posting Range: USD $47.37/Hr. - $63.16/Hr.
Your final compensation offer will be determined based on factors such as skills, experience, education, and licenses/certifications. The pay range is expressed as an hourly rate regardless of FLSA status. Exempt positions are paid salaried and are not subject to overtime.
