Job summary

Stay Graceful is seeking an in-home care provider to support clients in their own homes. The role emphasizes compassionate, respectful service and adherence to care plans under state regulations.

On-call rotations include one night per week and every 3–4th weekend, with ongoing documentation and team collaboration. The successful candidate will have strong communication skills, reliability, and a valid South Dakota driver’s license, with no degree required but experience in human services

Responsibilities

• Provides direct supervision and daily care of Stay Graceful clients in their home environment.
• Maintains a professional and caring relationship with clients; sets boundaries as needed.
• Plans and participates in activities per the care plan; attends team meetings and in-service hours.
• Maintains professional, courteous relationships with clients’ families and supporters.
• Adheres to SOPs and South Dakota regulations for in-home care.
• On-call: 1 night per week and 3–4th weekend; completes required documentation.

Responsibilities:

• Provides direct supervision and daily care of Stay Graceful clients primarily in their home environment.


• Implements and maintains a professional and caring relationship with all clients; providing reliable care, limit setting and boundaries while demonstrating understanding of the clients’ needs at all times.


• Plans and participates in activities with the clients as outlined in the care plan. ? Attends and actively participates in weekly team meetings, group supervision, and completes in-service hours annually as required by state regulations.


• Maintains a professional, courteous and helpful relationship with clients family and other primary social supporters.


• Adheres to Stay Graceful’s Standard Operations Procedures and South Dakota State Regulations/Administrative Rules relating to in-home care.


• Assures all clients, coworkers and family members are treated equally and with respect. ? Responsible for the quality of services and completion of necessary in-home care duties and related training including:

* Medication Management

* CPR training

* First Aid training

* Training in life skills and other care techniques and adherence to these techniques

* Daily care support may include assisting clients in household cleaning, laundry, cooking meals, grocery shopping, communicating with family and friends, and personal grooming.

• Completes and/or assures completion of necessary documentation and reports by the work day.


• Knowledgeable and reflect Stay Graceful’s Mission Statement, Core Values and Vision Statement.


• Each In-Home Care Provider is expected to meet Stay Graceful’s Code of Conduct, Professionalism, and Ethics.


• Other duties as assigned.


• On Call 1 night a week and every 3/4th weekend

Education and Experience Equivalencies:

• No degree required. Applicable experience strongly preferred in Human Services or a related field. Applicants with excellent abilities in communication via email, Word, and medical records preferred. All candidates must have (or qualify to obtain) a current South Dakota driver’s license, proof of insurance and be cleared through the Division of Criminal Investigation.

Physical Requirements:

• Ability to perform duties as outlined above including lifting 10-20 lbs.


• Continual standing and walking during shift.


• Continual talking in person or on the phone during shift.


• Ability to perform duties as outlined above including lifting 10-20 lbs.


• Will occasionally encounter toxic chemicals during shift.
