Primary Care Solutions is seeking a Licensed Clinical Social Worker to provide exceptional care to Veterans. You will conduct assessments and offer a full range of services, including counseling and resource referrals, at our clinics in New Mexico.

The ideal candidate is driven, with a Master's degree in Social Work and a valid LCSW license. This role emphasizes teamwork and patient care, ensuring timely interventions for Veterans facing varied psychosocial challenges.

Qualifications

• Master's degree or higher in Social Work from an accredited program.
• Active, unrestricted LCSW licensure for a minimum of 1 year.
• Minimum of 2 years post-master independent counseling experience with adults.
• Current certification in Basic Cardiac Life Support (BLS).
• Clinical practice onsite at one of the NM locations.

Responsabilités

• Conduct bio-psychosocial assessments for Veterans and their families.
• Provide clinical social work services including counseling and resource referrals.
• Participate in treatment planning with interprofessional teams.
• Assist with long-term care placements and discharge planning.
