Nuvance Health Med Practice CT is seeking an experienced LCSW to provide emotional and psychosocial support to patients and families within a hospital/crisis setting. You will deliver therapy, care coordination, discharge planning, and advocacy as part of a multidisciplinary team.

The role requires at least 3 years as an LCSW with strong clinical skills in risk assessment, crisis intervention, and evidence-based modalities such as CBT/DBT/EMDR.

Qualifications

• Requires at least 3 years of experience as an LCSW.
• Clinical background in hospital or crisis settings preferred.
• Certification or specialization in evidence-based interventions (CBT, DBT, EMDR) preferred.

Responsabilités

• Completes patient assessment and reassessment.
• Assesses risk and safety; recommends next steps.
• Provides counseling, individual, group, and crisis interventions.
• Completes required documentation for compliance.
• Collaborates with colleagues and psychiatric staff for treatment.
• Develops patient-focused treatment plans and discharge plans.
• Maintains knowledge of community resources and makes timely referrals.
• Applies evidence-based therapeutic practices.
