Job summary

Qualifications

• Assess patients' psychosocial needs through intensive interviewing and records review.
• Evaluate coping, support systems, and resources affecting care and discharge planning.
• Identify barriers and plan interventions to support goals of care and discharge.
• Provide crises intervention and counseling for individuals in temporary or situational distress.
• Refer patients to community resources for further intervention such as palliative or hospice services.

Responsibilities

• Communicate findings and plans to the interdisciplinary team and document interventions in medical records.
• Comply with reporting requirements in accordance with state law and hospital policy.
• Assist with arranging community resources and financial assistance as needed.
• Facilitate family meetings to support decision-making when goals are unclear.
