Vérité Health Collective is seeking a Licensed Clinical Social Worker to lead behavioral health intakes and care coordination across 8–10 post‑acute and long‑term care facilities in a regional territory. You will conduct assessments, evaluate patients for behavioral health and Value‑Based Care programs, and mentor field Coordinators.

Competenze

• Active LCSW license in good standing.
• Experience with psychiatric intakes and psychosocial assessments.
• Experience in behavioral health case management or care coordination.
• Familiarity with post-acute/long-term care or geriatric populations is plus.

Compensation: $80,000–$95,000 OTE/year

Location: Regional Territory (Naples, FL) – travel to 8–10 post‑acute/long‑term care facilities

Job Type: Full‑Time

The Role

We're hiring a Licensed Clinical Social Worker to lead behavioral health intakes and care coordination across a regional network of 8–10 post‑acute and long‑term care facilities. You'll conduct assessments, evaluate patients for behavioral health and Value‑Based Care programs, and support the clinical development of Behavioral Health Coordinators in the field.

What You'll Do

• Conduct psychiatric intakes and psychosocial assessments for diagnosis and treatment planning.
• Evaluate patients for eligibility across behavioral health and Value‑Based Care programs; obtain informed consent.
• Educate patients, families, and facility staff on available behavioral health services.
• Coordinate referrals, transitions of care, and handoffs across the care continuum.

What You Bring

• Active LCSW license (or equivalent independent clinical licensure) in good standing.
• Experience with psychiatric intakes, psychosocial assessments, or behavioral health case management.
• Comfort with EHR systems and digital communication tools.
• Experience with post‑acute, long‑term care, or geriatric populations a plus.
• Reliable transportation and willingness to travel regionally.
