Veritehealthcollective is seeking a Licensed Clinical Social Worker to lead regional behavioral health intakes and care coordination across 8–10 post-acute and long-term care facilities in a regional territory around Miami, FL. You will conduct assessments, evaluate eligibility for behavioral health programs, and support the clinical development of coordinators in the field.

The role requires an active LCSW license, experience with psychiatric intakes, and comfort with EHR systems.

Qualifications

• Active LCSW license in good standing.
• Experience with psychiatric intakes and psychosocial assessments.
• Comfort with EHR systems and digital communication tools.
• Experience with post-acute, long-term care, or geriatric populations is a plus.
• Reliable transportation and willingness to travel regionally.

Responsibilities

• Conduct psychiatric intakes and psychosocial assessments for diagnosis and planning.
• Evaluate patients for behavioral health and Value-Based Care programs; obtain consent.
• Educate patients, families, and facility staff on services available.
• Coordinate referrals and transitions of care across the care continuum.
• Serve as clinical escalation point for complex cases and conflicts.
• Maintain compliant EHR documentation and verify accuracy.
• Mentor and train Behavioral Health Coordinators, including onboarding new staff.
• Flag operational and staffing barriers to leadership.
• Support audits, quality reviews, and compliance initiatives.
• Travel across a defined region to support 8–10 facility sites.

Compensation: $80,000–$95,000 OTE/year

The Role

We're hiring a Licensed Clinical Social Worker to lead behavioral health intakes and care coordination across a regional network of 8–10 post-acute and long-term care facilities. You'll conduct assessments, evaluate patients for behavioral health and Value-Based Care programs, and support the clinical development of Behavioral Health Coordinators in the field.

What You'll Do

• Conduct psychiatric intakes and psychosocial assessments for diagnosis and treatment planning.

• Evaluate patients for eligibility across behavioral health and Value-Based Care programs; obtain informed consent.

• Educate patients, families, and facility staff on available behavioral health services.
