Job summary

STGi is seeking a Licensed Clinical Social Worker (LCSW) or Licensed Professional Mental Health Counselor (LPMHC) to deliver social work services at a Community Based Outpatient Clinic (CBOC) under VA guidance. The role involves assessment, treatment planning, referrals, and collaboration with a multidisciplinary team to support adult and geriatric patients.

Responsibilities include crisis intervention, discharge planning, and ensuring accurate documentation for quality care and continuity

Qualifications

• Must hold an LCSW or LPMHC license.

Responsibilities

• Provide social work services per physician orders to address patients' physical, mental, and emotional needs.
• Perform initial social work assessments for adult/geriatric primary care and/or psychiatric patients.
• Assist in treatment planning, discharge planning, referrals and placements as applicable.
• Provide coverage for chemically dependent patients as needed.
• Offer psychotherapeutic interventions for complex psychosocial issues.
• Make referrals for individual, group, family, and other therapies as appropriate.
• Maintain confidentiality and complete required documentation in medical records and VISTA.
