Job summary

Rehab Without Walls Neuro Rehabilitation seeks a Licensed Clinical Social Worker (LCSW) for Home & Community, full-time, to conduct psychosocial evaluations and develop individualized goals with the rehab team. This role provides 1:1 counseling to patients and families in home and community settings.

You will collaborate with PT/OT/SLP, support community reintegration, monitor progress, and advocate for independence and improved quality of life.

Qualifications

• Master's Degree in Social Work or related field as required.
• Current licensure/certification in state of practice (LCSW/LSW or equivalent).
• Minimum 1 year of rehab setting experience.
• BLS First Aid and CPR certifications.
• Excellent interpersonal, verbal, and written communication skills.
• Ability to work independently and as part of a team.
• Strong organization and attention to detail.

What you’ll do:

• Complete psychosocial evaluations and develop individualized goals alongside the rehabilitation team
• Provide 1:1 counseling and social work interventions for patients and families
• Help patients navigate emotional adjustment, relationships, independence, and community reintegration

• Collaborate closely with PT, OT, SLP and other members of our interdisciplinary team

Responsibilities

• Conducts comprehensive evaluations, assesses the social, emotional and support needs of patients and their families, and sets measurable objectives and goals that are formulated in conjunction with the rehabilitation team. Adjusts treatment plan to achieve maximum results in independence and/or function.
• Identifies and assists the patient and/or family to utilize appropriate community resources to achieve measurable objectives. Utilizes special skills and judgment to resolve adverse social and emotional responses connected with the injury/illness.
• Provides direct social work intervention and instructs and counsels the patients and family members in treating and coping with social and emotional responses connected with the injury/illness.
• Regularly re-assesses effectiveness of treatment plan, progress of patient towards goals and the need for modifying goals or treatment.
• Communicates patient’s needs and progress to the treatment team, physician, patient, family members, and others as applicable.
• Consults with medical providers regarding patient evaluation & treatment, progress of the patient &discharge from treatment.
• Observes/documents and reports changes in the condition/status of the person receiving services to the Clinical Coordinator, Program Manager or DCM, attending physician and other treatment team members.
• Maintains and submits evaluation, treatment and billing documentation in the format and timelines required by the Company, facility, funding source, local, state and federal regulations.
• Adheres to standards of the Company's Code of Business Conduct, policies, HIPAA and Corporate Compliance programs and appropriate professional practice standards and applicable state/federal laws.
• Performs other duties as assigned.

Qualifications

• Master's Degree in Social Work or Marriage and Family Therapy (LCSW, MSW, MFCC, or LPC)
• Current Social Work or Marriage and Family Therapist licensure/certification in the state(s) of practice required by individual states.
• A minimum of one year of experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or other acute diagnosis.
• Current BLS (Basic Life Support) First Aid and CPR certifications.
• Demonstrates effective and professional interpersonal, verbal, and written communication skills.
• Able to work independently and part of a team.
• Demonstrates strong organization skills and attention to detail.
