Job summary

Collage Home and Community Rehabilitation Partners is seeking Licensed Clinical Social Workers to provide patient-centered care in Chino, CA and surrounding areas in a home-based setting. PRN positions are available for clinicians who will accept patients for the duration of their stay.

Interdisciplinary teams create integrated treatment plans, focusing on functional goals and durable outcomes, with services provided in patients' homes and communities.

Qualifications

• Completion of a Master's degree in Social Work and state licensure as an LCSW are required.
• Excellent planning, organization, writing, and time management skills are required.
• Ability to communicate with families, professionals, and co-workers is a must.

Responsibilities

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification
