Job summary

Collage Home and Community Rehabilitation Partners seeks an experienced Licensed Clinical Social Worker to provide patient-centered care in Lewisville, TX and surrounding areas. PRN positions are available for clinicians who will accept patients for the duration of their stay.

Our interdisciplinary teams create integrated treatment plans delivered in patients' homes and communities, addressing neurological injuries and related conditions with a flexible, evidence-based approach.

Qualifications

• Master's degree in Social Work and state licensure as an LCSW are required.
• Excellent planning, organization, writing, and time management skills are required.
• Ability to communicate with families, professionals, and co-workers is a must.

Responsibilities

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification
