Job description

A community-focused healthcare organization in Gilroy, California, is seeking a full-time LCSW or Psychologist to provide integrated behavioral health services and clinical consultation within a primary care setting.

Position Highlights:

• Provide integrated behavioral health services to primary care patients with mental health needs
• Collaborate with primary care providers on psychological disorders, substance use disorders, chronic pain, chronic disease, and related psychosocial concerns
• Conduct behavioral health screening, triage, evaluation, and assessment
• Provide individual, family, and group interventions using evidence-based therapeutic methods
• Provide crisis intervention, ad hoc counseling, treatment planning, and behavioral health interventions
• Provide psychopharmacological consultation to primary care providers in collaboration with a psychiatrist and within scope of practice
• Provide behavioral health consultation and training to clinic staff
• Work closely with medical case managers and primary care providers
• Educate and support patients and families regarding medical and behavioral health needs
• Coordinate referrals to appropriate community resources
• Provide comprehensive behavioral health services including intake evaluations, assessments, collateral interventions, outreach, and follow-up
• Maintain accurate patient records, clinical documentation, reports, protocols, and program materials
• Participate in case conferences, peer review, quality assurance, and program evaluation
• Support a culturally responsive approach to care for a diverse patient population

Qualifications:

• Master’s degree in social work/social welfare or doctoral degree in Clinical or Counseling Psychology
• Current California LCSW or Psychologist license
• Current LPCC license
• One year of directly related experience preferred
• Experience in primary care consultation, behavioral medicine, health psychology, or a related field preferred
• Experience in public/community health settings and/or serving low-income multicultural populations preferred
• Spanish bilingual skills highly desirable

Compensation & Benefits:

• $92,200–$123,338 annual salary
