Job summary

Strive Health in Wisconsin is seeking a Licensed Clinical Social Worker (LCSW) to provide psychotherapy and psychosocial support to residents in skilled nursing facilities. The LCSW conducts assessments, delivers individual and group therapy, and collaborates with the interdisciplinary team while ensuring regulatory compliance.

Applicants must hold an MSW and an active Wisconsin LCSW license, with preference for geriatric or long‑term care experience.

Qualifications

• Master’s in Social Work (MSW) with active LCSW license in Wisconsin.
• Enrolled or eligible for enrollment as a Medicare Part B provider.
• Geriatric or long‑term care experience preferred.
• Strong interpersonal, documentation, and communication skills.
• Ability to work independently within a collaborative, team‑based environment.

Responsibilities

• Conduct initial assessments and develop individualized treatment plans.
• Provide individual and group psychotherapy.
• Deliver family counseling and support for residents.
• Document sessions in compliance with Medicare and facility requirements.
• Participate in care plan meetings and IDT discussions.
• Support residents and families with coping strategies for illness and placement.
• Collaborate with Strive practitioners and facility staff for coordinated care.
• Educate facility staff on recognizing psychosocial needs and referral processes.
