Impact Health PC seeks a full-time Licensed Clinical Social Worker (LCSW) to provide comprehensive mental health services to residents of skilled nursing facilities in the Augusta, Maine area. You will conduct assessments, deliver individual therapy, and collaborate with a multidisciplinary team, ensuring timely documentation and compliance with state regulations.

Qualifications

• Active Maine LCSW license.
• At least 2 years of clinical experience providing mental health care to adults.
• Experience in skilled nursing facilities or geriatric mental health.
• Strong communication, assessment, diagnostic, and psychotherapy skills.

Responsibilities

• Provide person-centered mental health care using evidence-based practices.
• Conduct assessments and provide diagnostic and psychotherapy services.
• Meet with residents individually for therapy sessions.
• Manage a caseload and address residents’ mental health needs.
• Coordinate care with facility staff and Impact Health’s clinical team.
• Collaborate with medical and psychiatric providers.
• Complete documentation within 24 hours of each encounter.
• Follow applicable state regulations and skilled nursing facility requirements.
