Job summary

The ideal candidate will have an MSW and Maryland LCSW-C licensure, with 2+ years of relevant experience in integrated healthcare.

Qualifications

• MSW from an accredited program is required.
• Active LCSW-C license in Maryland is required; LMSW may be considered if licensure is in progress.

Responsibilities

• Provide behavioral health interventions, care coordination, case management, and patient advocacy.
• Coordinate with primary care teams to support high-risk and medically complex patients.
• Engage patients in treatment planning and collaborative care with PCPs.
• Assist with insurance and benefits navigation, including Maryland Medicaid and Medicare.
