Job summary

Qualifications

Responsibilities

• Collect and document data within 24 hours of admission.
• Document and report significant findings of ongoing patient assessment.
• Formulate the patient plan of care based on standards of practice.
• Communicate the patient plan of care through shift reports and procedures.
• Plan individualized patient care with nursing theory in collaboration with patients and providers.
• Initiate patient plan of care using admission assessment data.
• Incorporate nursing diagnoses into the care plan.
• Implement the plan of care with patients, family, and others.
• Formulate discharge plans within 24 hours of admission and revise as needed.
• Evaluate nursing interventions based on patient outcomes.
