Qualifications

Responsibilities

• Assess patients’ physical, social, and psychological status and gather information from the interdisciplinary team.
• Plan individualized care with patient and family involvement and coordinate education and follow-up.
• Implement care and coordinate with the care team, managing priorities, time, supplies, and resources.
• Evaluate patient responses, revise the care plan as needed, and ensure timely documentation.
• Maintain professionalism and compliance with licensure and regulatory standards.
