Job summary

Mass General Brigham Home Care, Inc. is seeking a Certified Nursing Assistant/Patient Care Technician to provide direct patient care and support to the nursing staff in a hospital setting.

You will assist with daily living activities, monitor patient conditions, and help maintain a safe, respectful environment for patients. The role requires a High School Diploma or equivalent, CNA/PCT certification, and at least two years of CNA experience.

Qualifications

• High School Diploma or equivalent required.
• 2 years of CNA experience in a healthcare setting.
• Certified Nursing Assistant/Patient Care Technician (CNA/PCT) certification required.

Responsibilities

• Assist patients with activities of daily living (bathing, dressing, grooming, toileting, mobility).
• Provide basic care and comfort, including feeding and repositioning patients.
• Assist with intake and discharge procedures, room prep, and comfort measures.
• Collect and document patient information (intake/output) and assist with specimen collection as needed.
• Communicate effectively with patients, families, and healthcare team members regarding needs and plans.
• Document care activities and changes in the EHR or paper charts.

• Help with patient intake and discharge procedures, including preparing rooms and providing comfort measures.
• Collect and document patient information, including intake and output, and assist with specimen collection as needed.
• Communicate effectively with patients, families, and healthcare team members regarding patient needs and care plans.
• Document patient care activities, observations, and changes accurately and timely in the electronic health record (EHR) or paper charts.
