Job summary

Theoria Medical is currently seeking an Advanced Practice Provider to join their team serving local skilled nursing facilities. The role includes delivering patient-centered care and managing treatment plans, offering flexibility in scheduling and a competitive compensation package.

You will have access to technology designed to streamline workflows and an opportunity for annual bonuses as well as incentives. Ideal candidates will hold a valid U.S. license and be board-certified in their specialty.

Qualifications

• Must be an Advanced Practice Provider with a valid U.S. license.
• Board certification is required for Nurse Practitioners or Physician Assistants.
• Experience in Internal Medicine, Family Medicine, or Geriatric Medicine is preferred.

Responsibilities

What You’ll Do

• Provide high-quality, patient-centered care to residents in skilled nursing facilities
• Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues
• Communicate with facility nursing staff via phone or secure messaging as needed
• Maintain reliable on-site attendance for scheduled shifts
• Ensure compliance with clinical standards, facility policies, and regulatory requirements

Your Qualifications

Advanced Practice Providers must hold an active U.S. license and be board-certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants). Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
