Sterling St James Inc. on Long Island is seeking an Internal Medicine Nurse Practitioner to deliver high-quality outpatient care in a busy physician-led group.

The role focuses on preventive, acute, and chronic care with emphasis on patient education and collaboration with physicians. Qualifications

• BSN and MSN from an accredited nursing school.
• NYS RN license and NP license, BLS and Board Certified.
• Trained in Internal Medicine.
• 8+ years experience as a NP.
• Strong diagnostic, communication and interpersonal skills.
• Team-oriented with a patient-centered approach to care.
• Proven ability to maintain and grow a local clinical practice is required.
• Entrepreneurial mindset with ability to grow and manage a thriving patient base.
• Familiarity with EMRs and value-based care metrics is a plus.

Responsabilités

• Serve as a key clinical provider in the delivery of high-quality care.
• Provide direct patient care in an outpatient primary care setting, including preventive, acute, and chronic disease management.
• Refer patients to appropriate specialty services as needed.
• Provide clear communication and education to patients regarding diagnoses, treatment options, and preventive strategies.
• Maintain thorough and accurate documentation within the EMR system.
• Collaborate with referring physicians, specialists, and care teams to ensure seamless patient care.
• Provide clinical oversight to nurses, technologists, and support staff within the practice.
• Stay compliant with HIPAA, NYS DOH, CMS, and relevant payer guidelines.
• Participate in population health initiatives and support ongoing quality improvement projects.
• Contribute to the development of a sustainable patient panel aligned with practice growth objectives.
