Interim Physical Therapist (13 Week Contract)

Position Overview

Bedrock Healthcare Center prioritizes caring for seniors with respect, compassion, and dignity. Our dedicated nursing staff maintain high professionalism to help residents regain independence in a friendly environment. We are seeking a Physical Therapist to join our team.

Sign-on Bonus: $10,000!

Purpose of the Position

Evaluate, treat, document, and facilitate care for residents with impaired ability to function due to disease; participate in interdisciplinary team meetings; assist in developing decline prevention policies.

Key Responsibilities
  1. Evaluate and document resident assessments per facility policies.
  2. Provide treatments to help residents maintain or improve function.
  3. Recommend and provide preventative measures and adaptive equipment.
  4. Maintain accurate reports for regulatory agencies and payers.
  5. Observe infection control and safety procedures.
  6. Complete Minimum Data Set assessments as assigned.
  7. Document evaluations, treatment plans, and progress notes timely.
  8. Participate in care conferences and quality improvement programs.
  9. Collaborate effectively with residents, families, staff, and healthcare providers.
  10. Adhere to all policies, procedures, and confidentiality requirements.
  11. Maintain professional appearance and punctuality.
  12. Perform other duties as assigned.
Qualifications
  • Excellent verbal and written communication skills.
  • Ability to analyze medical information and communicate effectively.
  • Proficiency in Word and Excel.
  • Doctorate of Physical Education or equivalent.
  • Active Physical Therapist Licensure.
