Job summary

Prestige Care Center of Plattsmouth is seeking an experienced MDS Coordinator Registered Nurse to join our caregiving team. You will be responsible for accurate MDS documentation and care planning to support resident health and healing.

As an in-house case manager, you will communicate with residents, families, and care professionals, ensuring adherence to Medicare, Medicaid, and other payer requirements, and performing thorough triple checks.

Qualifications

• Experience with MDS, Care Area Assessments, and care plans.
• Strong regulatory knowledge relevant to Medicare/Medicaid.
• Ability to communicate effectively with residents, families, and care teams.

Responsibilities

• Accurate and thorough completion of the MDS, Care Area Assessment, and Care Plans.
• Serve as in-house case manager for residents.
• Communicate resident status and care needs to families, other health professionals, and third-party payers.
• Ensure compliance with Medicare, Managed Care, and Medicaid requirements.
• Perform Triple Check for timely documentation.

Accurate and thorough completion of the MDS, Care Area Assessment, and Care Plans, in accordance with current federal and state regulations and guidelines that govern the process.

Acts as an in-house case manager demonstrating detailed knowledge of residents' health status, critical thinking skills to develop appropriate care pathways, and timely communication of needed information to the resident, family, other health care professionals, and third-party payers.

Demonstrate an understanding of MDS requirements related to varied payers including Medicare, Managed Care, and Medicaid.

Responsible for timely and accurate completion of Triple Check.
