Job summary

One Hope United is seeking an IPS Aftercare Specialist in Chicago to provide intensive stabilization and post‑inpatient transition support for youth. You will assess mental health needs, diagnose when appropriate, and coordinate ongoing therapeutic services with caregivers, schools, and community partners.

Serve as a liaison across clinical teams, maintain secure records, and use evidence‑based modalities and CANS/IM CANS. Travel regionally as required and mentor interns and staff.

Qualifications

• Master’s degree in social work or related field.
• 2–5 years of relevant experience as a licensed professional or license-eligible.
• Current licensure or eligibility (LCSW/LCPC/LSW/LMLP equivalent).
• Experience with CANS/IM CANS and ARC framework.

Responsibilities

• Provide intensive crisis intervention and aftercare services.
• Conduct initial visit within 5 days of referral.
• Coordinate services across office, home, and community.
• Maintain confidential client records and reporting obligations.
• Lead staff and interns, mentor new counselors.
• Administer C-SSRS to youth aged 7–21 as needed.
• Document SDS notes and monthly progress reports.
• Collaborate with caregivers, agencies, and providers.
