Job summary

Easterseals VT is seeking an Intensive Family Based Services Coordinator to join our team. The role involves writing care plans, notes, and reports, while coordinating with family service workers and other providers to support families in home and community settings.

You will help families access housing, transportation, counseling, and other resources, while guiding caregivers in meeting children's needs and safety. Flexible in-home support and court testimony may be required.

Qualifications

• Bachelor's degree in a related field.
• Two years of experience preferred.
• Excellent writing skills necessary.
• Valid driver's license and reliable transportation.

Responsibilities

• Write and implement plans of care approved by the consulting agency clinician.
• Write contact notes, monthly reports, and discharge summaries.
• Maintain frequent communication with Family Service Workers to ensure compliance with the family’s case plan.
• Provide services to families within their home and community settings.
• Assist families in finding resources for housing, transportation, substance abuse treatment, counseling and nutrition.
• Help caregivers recognize children’s needs and implement strategies for growth and safety.
• Attend meetings, trainings, and related events as assigned.
• Provide transportation for clients when required.
• Testify concerning services in court when subpoenaed.
• Ensure Family Assessments are completed in a timely manner per IFBS policies.
• Create crisis plans and respond to crisis calls as needed.
• Provide in-home family support to fit family schedules.
• Implement short-term 13-week interventions to prevent DCF involvement or support reunification.
• Coordinate services with Schools, Early Intervention, clinicians and other providers.

We are looking for an Intensive Family Based Services Coordinator to join our Easterseals VT team! The individual in this key position will be responsible for the following:

• Write contact notes, formal monthly reports and discharge summaries.


• Maintain frequent communications with Family Service Workers to ensure compliance with the family's case plan.


• Provide services to families within their home and community settings


• Assist the family in finding resources for preventative and supportive services ie; housing, transportation and substance abuse treatment, counseling and nutrition.


• Help caregivers to recognize their children's needs, and make plans and implement strategies so that they meet their children's needs and grow in their confidence and skills, while providing a safe environment for their children.


• Participate in meetings, conferences, trainings events as assigned.


• Provide transportation for clients when required.


• Testify concerning services in court when subpoenaed.


• Ensure Family Assessments are done within a timely manner as prescribed by IFBS policies


• Create crisis plans and respond to crisis calls as needed


• Provide in home family support during the times that work for the family.


• Implement short term (13 week) intervention and support in an effort to prevent DCF involvement or out of home care, and/or to reunify children with their family; to improve family functioning.

Compensation: Starting $23.50 per hour. Additional compensation based upon years of related experience.

Hours: This is a full-time, 40 hour/week position. M-F 8:30-5:30
