Job summary

Wellstone in Huntsville, AL is seeking an Experienced Intensive Care Coordinator to support youth and young adults through strengths-based plans within the NOVA Services Program. The role requires completing assessments (CANS), developing service plans, and coordinating with families and community providers.

Work hours are Monday–Friday, 7:30 AM–4:30 PM, with up to 50% travel. You will document progress, advocate for clients, participate in crisis and backup planning, and ensure accurate

Qualifications

  • Bachelor of Arts/Science in a related field.
  • Valid Alabama driver's license and a suitable automobile.
  • Excellent verbal and written communication skills.
  • Certified as a Care Coordinator or ability to obtain certification.
  • Prefer experience with children/adolescents with mental health issues.
  • Extensive knowledge of the community resources.

Responsibilities

  • Complete strength-based and needs-based assessments and implement service plans.
  • Promote involvement of youth, parents and community allies; arrange necessary supports.
  • Liaise between youth, family and service providers to ensure coordinated care.
  • Maintain accurate records, releases, demographics, and paperwork.
  • Document progress in electronic charts and prepare reports.
  • Support families emotionally and practically and collaborate with the NOVA team.
  • Develop crisis and backup plans and provide transportation when needed.
  • Ensure services are meaningful, respectful and promote self-determination.

Job Description

The Intensive Care Coordinator will provide direct care for the clients of the NOVA Services Program. This position addresses the critical domains of functioning for youth/young adults, to include working, learning, living, belonging and healing through a strengths-based individualized plan.This position will be working Monday - Friday, 7:30AM - 4:30PM.

What you will be doing:
  • Complete strength-based and needs-basedassessments, including the CANS, and effectively implement written service plans based on the assessments to meet individualized goals and objectives.
  • Promote the involvement of youth/young adults, parents and other natural/community allies. Arrange and provide necessary supports and services.
  • Serve as a liaison between the youth/young adult, their family members and various service providers to ensure a community of care.
  • Ensure that all needed information is on file and accurate, including Releases of Information, demographic data, initial paperwork, financial information, etc.
  • Document information per standards in electronic client chart, i.e. treatment plans, progress notes, referrals, etc. in a timely andaccurate manner.
  • Work alongside families/care givers in providing information and supporting them emotionally and practically.
  • Collaborate with NOVAteam members to ensure that all goals are being achieved.
  • Ensure that all services provided are meaningful, respectful and promote self-determination.
  • Participate in the development of crisis and backup support plans for each youth/young adult.
  • Provide transportation when necessary.
  • Provide accurate information, both verballyand in writing, for daily, weekly and monthly reports/meetings as stipulated by program demands.
  • Complete other duties as assigned.

Qualifications
  • Bachelor of Arts/Science in a related field.
  • Valid Alabama driver's license and a suitable automobile to permit the required travel. Must show car insurance equal to or in excess of the State of Alabama standards for automobile insurance coverage.
  • Excellent verbal and written communication skills.
  • Either certified as a Care Coordinator (adults and children) by ADMH-approved trainings or ability to obtain such certification.
  • Prefer experience with children/adolescents, especially those with serious ongoing mental health problems and those of challenged backgrounds.
  • Extensive knowledge of the community including resources.
