Intensive Care Case Manager

Status: Non-Exempt

Department: Empowerment Programs

Reports To: Clinical Supervisor

Location: Los Angeles, CA / On-Site

Summary of Position

The Intensive Care Case Manager provides intensive case management, client support, and crisis intervention to survivors of human trafficking. The Intensive Care Case Manager will provide services for domestic and foreign national victims of human trafficking, and will primarily serve minors, transition-age youth (TAY), as well as adults.

This position requires thorough knowledge and understanding of a human rights, strengths-based approach in serving clients and ability to employ key social work values, including client empowerment, professional boundaries, and self- care. The candidate should also possess client management expertise, independent decision-making ability, and excellent interpersonal and team-building skills. This position requires field work and travel throughout Los Angeles County, develop and work with partners and to provide services to trafficking survivors in the Los Angeles region. The Intensive Care Case Manager also supports CAST's 24-hour Emergency Response (ER) program, as a member of the on-call team.

Essential Responsibilities

Case Management: Provide survivor-driven, client-centered timely and professional case management for domestic and foreign national victims of human trafficking.

  • Provide initial screening and assessment of new referrals;
  • Provide follow-up intensive care with minor/TAY referrals to maintain engagement following initial emergency response;
  • Manage initial intake for new clients, including initial safety and service planning;
  • Provide on-call 24-7 support to intensive care clients for first 90 days; For non-intensive care clients, provide crisis intervention and supportive counseling as needed;
  • Provide ongoing service planning with clients and maintain written records of all service plans and achievements;
  • Assist client to coordinate, health, mental health, legal, employment, education, criminal/civil case, DPSS appointments, additional needs that are assessed, and accompany/drive clients as ;
  • Assess the need for and distribute emergency funds and resources to clients;
  • Complete outcome measures and evaluations;
  • Manage case files and provide detailed progress notes in hard copy files and in database on a daily basis; and
  • Update holistic client assessments at least quarterly, including in hard copy client files and

Youth Event Planning: Help in planning youth events throughout the year.

  • This includes but not limited to attending event planning meetings, researching potential event sites, being available some weekends and evenings for events, working with vendors and/or businesses that support events, and coordinating transportation

Client Advocacy and Community Outreach: Ensure that client rights and needs are protected at local service delivery level and by service providers.

  • Identify barriers to services and advocate for higher quality of services with external service organizations;
  • Ensure protection of identity and confidentiality in line with survivors' safety, needs and preferences; and
  • Advocate for culturally and linguistically appropriate services for clients; and
  • Participate in outreach, including development and delivery of technical assistance and training to community and faith-based organizations, government, and law enforcement officials, and

Emergency Response/Crisis Intervention: As support for the Emergency Response (ER) team, provide 24-hour response to human trafficking reports from the community:

  • Provide remote emergency response services for Cast hotline inbound calls during scheduled working shift (up to 3 shifts a month, as requested by Emergency Response Program Manager when scheduling coverage is needed by the first responder team)
  • Perform client intake, screen new referrals, triage needs, create initial action plans, and coordinate with client, CAST staff, and partners to address emergency needs.
  • Provide victim service assistance to potential victims at law enforcement operations, or healthcare facilities
  • Support individuals calling Cast's 24/7 hotline during scheduled working shifts

Program Development:

  • Attend program meetings to discuss fundraising opportunities for the Youth Program as well as planning and developing new programming and events.
  • Other Duties: Provide supervisory and development support to the MSW Interns, and Perform other duties in line with Empowerment Program and/or CAST goals, as identified and assigned by the Clinical Supervisor, Emergency Response Program Manager and/or the Director of Empowerment Programs.
Qualifications/Requirements
  1. Preferred but not required a S.W., BA in social work or related field or M.S.W. or MA
  2. Bilingual preferred (Spanish or other languages).
  3. Demonstrated sensitivity to and knowledge of issues involved in working with diverse populations and/or with individuals who have limited English proficiency.
  4. Demonstrated experience (minimum two years) in case management and/or providing direct services to clients, preferably in a community-based organization.
  5. In depth knowledge of issues related to providing services to survivors of human trafficking and other vulnerable populations, including knowledge of the service delivery system in Los Angeles and the regulations governing public and private benefits as well as federal benefits available to trafficking survivors.
