Centerstone Medical Group is seeking a Physician – Primary Care to deliver comprehensive, integrated care in a Federally Qualified Health Center setting in Hannibal, MO and Quincy, IL. You will work with a multidisciplinary team and provide medical evaluations, management of chronic conditions, and care coordination with behavioral health services.

Qualifications

• MD or DO from an accredited institution.
• Board Certified (or Board Eligible) in Family Medicine or Internal Medicine.
• Active (or eligible) Missouri or Illinois medical license.
• DEA license.

Responsibilities

• Provide comprehensive primary care with evaluations, diagnosis and treatment planning.
• Manage chronic conditions and co-occurring behavioral health and substance use needs.
• Ensure continuity of care through follow-up and coordination with the care team.
• Collaborate with behavioral health providers, care managers, and support staff.
• Participate in case reviews and care planning discussions.
• Contribute to program development, quality improvement, and care model advancement.
