• Reliable transportation may be required.

Responsibilities

• Provide counseling services and coordination of care for patients and families.
• Maintain a caseload of up to 75 patients.
• Conduct intake assessments and participate in Child and Family Teams.
• Create individualized service plans and monitor progress.

Skills

BLS certification
